1,000-Lb. Sisters star Tammy Slaton bids an emotional farewell to husband Caleb Willingham as she prepares to leave the weight rehab clinic where they first met. In an exclusive first look at the TLC series' season 5 premiere, set to air on Tuesday, December 12, Caleb shares a heartfelt poem that he wrote for Tammy before her departure from the Ohio facility.
As Caleb reads the moving ode, "Sunflower, sunflower, God planted you as a seed in a field full of many, but you he meant for me," Tammy wipes away tears. He pauses to collect himself, overcome with emotion, as the words of the poem reach deep within.
“I love it,” Tammy tells Caleb after he finishes reading.

“You’re my sunflower,” he replies.
Following their long embrace, Caleb reassured Tammy that he would be joining her outside the clinic soon, promising, "I'm coming home, baby. I don't know when or how, but I'm getting there." Tammy nodded, resting her head on Caleb's shoulder.
The couple had tied the knot in November 2022 at the rehab facility where they first met a year earlier. Both had been receiving treatment for obesity-related health issues at the clinic. Tammy had successfully undergone gastric bypass surgery at the treatment center in the summer of 2022 and has since shared her remarkable weight loss journey on social media.
Less than a year after their wedding, Tammy announced on Instagram in July that Caleb had passed away at the age of 40. She wrote, "Rest in peace sweet angel. You will always be missed and loved so much. Thank you Caleb for showing me true love and happiness. ❤️️"
Two days after Caleb's death, Tammy mentioned that she was experiencing the "stages of grief." "Sure, we were facing difficulties, but I truly loved him and still do," she expressed tearfully in a TikTok video. "Despite everything, I believe he's in a better place now and free from suffering."
In a preview of the season premiere of 1,000-Lb. Sisters, Caleb expresses his worry about not having sufficient time with Tammy. "I have the love of my life right here with me and now I’m losing it," he shares with the cameras. "The biggest fear I have right now is not knowing our future. I’m happy for her, but it’s scary."
Despite having to say goodbye, Caleb and Tammy assure each other of their love.
“I love you. I do,” Tammy says in the clip while wiping away Caleb’s tears.
