Throughout the film, Monk considers suicide using his medication. After solving the murder case, Trudy helps him see his worth beyond being a detective. He changes his mind with the help of Natalie, who gives him a support dog.

The first indication of Monk's transformation is when his publishers drop his book deal. Trudy's unexpected appearance in the hallway makes it clear that something is amiss, especially since Monk had let her go during the series finale. He opens the window before Trudy tells him, "You need to stop thinking about that."

Monk, much older now, successfully weathered the pandemic with the support of Molly. The experience of the pandemic has changed him, just like many others. When Molly and Monk go to the airport to pick up Natalie and Randy, she admits that the pandemic has taken a toll on Monk, and he is starting over. He didn't leave the house for two years.

As the story unfolds, there are more signs that Monk is considering ending his life. His medicine cabinet is filled with multiple full bottles of his prescription. He tells the delivery boy that it will be the last refill for a while. On his calendar, a specific date is marked with Trudy's name. At one point, Monk is at the park near the bridge where he proposed to Trudy, and he tells her that it won't be long until he's with her again, which scares her.

Dr. Bell realizes during a session that Monk is reconnecting with Trudy. Meanwhile, while investigating threats to a local dog shelter, Natalie and Monk come across a blind dog who enjoys arranging his cage. Monk becomes despondent and tells Natalie that he believes he is unloved and should be put down, alluding to himself. Then, unable to find evidence of Rick Eden's guilt in Griffin's death, Monk takes decisive action, including writing letters to everyone, organizing pills, and even visiting a funeral home. However, Monk experiences an epiphany and successfully solves the case.

Undeterred, he continues his stroll in the park, accompanied by his medication and water. Trudy reassures him that he is not alone, and Griffin arrives to express his gratitude for Monk's help. More of Monk's saved victims soon show up to thank him. Molly finds him, and as he walks away, he is followed by those he has aided.

In the ending of Mr. Monk’s Last Case, Monk returns home to find a note from Natalie. She has given him a dog from the shelter, asking them to take care of each other.

However, I am thrilled to say that Mr. Monk's Last Case not only met my expectations but exceeded them.

The movie delves deeply into Adrian Monk's character, exploring his emotional struggles and personal growth in a way that is both poignant and heartwarming. Tony Shalhoub's portrayal of Monk is as brilliant as ever, and he brings a depth and nuance to the character that is truly remarkable. The supporting cast is also excellent, with Traylor Howard and Jason Gray-Stanford reprising their roles as Natalie Teager and Randy Disher, respectively.
