The Pale Elf comprises a lengthy quest chain, encompassing numerous steps. Prior to this stage, players must have successfully accomplished all the steps within Act 1 and Act 2 of the game. These steps are as follows: refrain from banishing or causing harm to Astarion when he attempts to consume the avatar's blood.

Eventually, the players will be informed by him that he is enslaved by a vampire named Cazador. He reveals that the presence of the Illithid tadpole is the sole reason why he hasn't been forced to comply with his master's commands.

To initiate a scene with him, escort him to the small camp located to the south of Auntie's Teahouse. There, they will encounter the monster hunter, who confirms his belief that Cazador is the individual responsible for hiring him.

Wait for a night cutscene where the player and Astarion interact. Astarion can be seen trying to trace the scars on his back with his fingers. Engage in a conversation about the scars and suggest the idea of trying to decipher or draw them in the dirt. Astarion will eventually discover that they are written in infernal script and request the player to bring him along for their next encounter with Rafael, as he wishes to inquire about them. In exchange for killing an infernal in the Mausoleum during Act 2, Rafael can decipher the text for them.

When Astarion discovers Cazador's intentions, he implores the players to accompany him to Baldur's Gate. His objective, whether the players intend to pursue a romantic relationship with this companion or not, is to eliminate Cazador. However, to locate him, Astarion requests the players to search for one of his vampire spawn slaves, whom he considers a "sibling".
