The Outlast Trials immerses players in a grueling "therapy" regime at the Murkoff Facility, where they must undergo a series of trials to earn the right to leave. These trials take place across four different maps and involve navigating deadly enemies, solving puzzles, and completing bone-chilling objectives.

As with previous Outlast titles, players are unable to engage in direct combat and must instead rely on stealth and evasion to survive. However, players can purchase various abilities and equipment to aid their efforts, including powerful prescriptions that grant special physical and mental enhancements. To acquire these valuable upgrades, players must seek out the assistance of the facility's resident nurse.

How to Get Prescriptions

Players arrive at the Sleep Room upon arrival at the Murkoff Facility, following the tutorial. This space serves as a hub where they can personalize their sleeping quarters, gain access to the main terminal to join trials, consult with an engineer, and visit the pharmacy. The pharmacy is managed by Emily Barlow, who offers prescribed items to players once they reach Therapy Level 3.

As players successfully complete trials, they earn XP that contributes to their Therapy Level. Although the trials are referred to as therapy, the prescribed items in The Outlast Trials are not used for medicinal purposes. Instead, they offer players enhanced abilities such as sliding, health regeneration, or the ability to defend against enemy attacks in co-op play. Regrettably, these items cannot be purchased with cash.

Prescriptions can be obtained by exchanging them for tickets earned by completing trials with a grade of D or higher. As players progress through the program, the trials become more difficult and offer multiple tickets as rewards. The prescription system consists of two tiers, with Tier Two only becoming available after all prescriptions in Tier One have been obtained.

Prescriptions, unlike the Rigs sold by the engineer, do not require cool-down periods and can be used at any time once purchased by players. They offer a range of effects, including increased strength, speed, stamina, inventory capacity, and more. While many prescriptions activate automatically, some require manual input, such as Slide, Doors Basher, and Instant Use.

The decision of which prescriptions to purchase first largely depends on a player's play style and whether they prefer to play solo or in multiplayer matches. Additionally, many prescriptions have supportive qualities that benefit both players and their friends, and when paired with Rigs, can create effective teams. However, players must purchase all Tier One prescriptions to access some of the best ones.
