Robert McCall, the protagonist of The Equalizer 3, concludes his gripping journey by eliminating the leader of the Italian mafia, ultimately choosing a tranquil retirement in Altamonte. Within the opening scenes, McCall strategically eradicates a mafia stronghold nestled within a Sicilian vineyard, all while clutching a mysterious bag whose contents remain undisclosed. Despite sustaining a gunshot wound during his daring escape, McCall successfully reaches Altamonte, an idyllic fictional sanctuary located in the southern region of Italy. Here, a benevolent local physician tends to his injuries and extends him a welcoming abode.
While exploring the town and forming relationships with café owners, policemen, and others, he reaches out to Emma Collins, a CIA officer specializing in financial crime, to share information about a potential illegal activity happening at the vineyard. They conduct an investigation and discover that the bottles contain drugs linked to terrorist funding in Syria.
Emma manages to locate McCall, but he remains secretive about his motives for contacting her. However, he hints at additional information regarding the town's situation. Eventually, Emma uncovers that the mafia, known as the "Camorra," is involved instead of a terrorist group in Italy. As she delves deeper into their operation, she narrowly escapes an assassination attempt with a car bomb.

In response, he eliminates a large squad of them, with their leader being no other than Vincent (Andrea Scarduzio), the brother of the mafia chief. To force the killer out of hiding, Vincent threatens to kill the local policeman. In a bold move, McCall appears in the town square, urging them to take him instead, while sparing everyone else from harm.
Vincent believes it is the perfect location to demonstrate his power, but unexpectedly, the entire town rises up against the mafia, proclaiming themselves as Spartacus. This sudden uprising terrifies the mob, causing them to retreat - at least for the time being. "I will be waiting here, so hurry," McCall urges Vincent, although he doesn't wait for long. McCall ruthlessly eliminates all of Vincent's guards at his residence, before incapacitating Vincent, administering the drugs they have been selling, and observing his agonizing crawl down the street until his heart ceases beating.
The following day, McCall pays a visit to Collins, who is hospitalized, carrying the same bag we witnessed at the beginning. Inside is approximately $360,000 in cash, which belonged to an average man in Boston, his savings having been stolen by a cyber criminal. When Collins questions why he assisted a stranger, he responds, "I once gave him a Lyft." He then departs, leaving the bag behind in the room as Collins returns to Boston to deliver the money. It is important to remember that The Equalizer operates covertly, ensuring that those who seek their aid are unaware that it is McCall they are reaching out to.
McCall sends Collins a package containing his little black book of government contacts, along with a note saying, "Your mother would be proud of you." Collins is actually the daughter of Susan and Brian Plummer, McCall's former colleagues from the DIA. Susan was tragically murdered in the previous installment. As the movie comes to a close, McCall is seen sitting at a cafe in Altamonte, enjoying a local celebration. He feels content and no longer needs to resort to violence. In fact, he is so relaxed that he even forgets his spoon on the table as he joins in on the fun. The story of The Equalizer comes to an end.
