Players are able to enhance their characters with a variety of Malignant Hearts, which are harvested from formidable new demons encountered throughout Sanctuary in the seasonal realm. These hearts have the ability to significantly enhance a character's core identity when utilized effectively.

Similar to socketable gems, Malignant Hearts can be inserted into a player's accessories and bestow powers comparable to those granted by Legendary Aspects. There are four categories of Malignant Hearts: Vicious (Offensive), Brutal (Defensive), Devious (Utility), and Wrathful (Super). Each category has unique effects, and they are further divided based on character class. However, most of the Malignant Hearts are classified as General, providing players with more freedom in buildcrafting. Blizzard has revealed all 32 Malignant Hearts for Diablo 4 Season 1.

General

The Picana (Vicious)Critical Strikes electrify the enemy for 0.75-2.50 seconds, creating a surge of lightning that arcs between them and any other electrified enemies, inflicting 68-136 Lightning damage.

The Lionheart (Brutal)

While your Life is above 60%, Core Skills now cost 51-68 Life instead of using your Primary Resource every 5 seconds. Skills that consume Life deal 10-20% more damage.

Tempting Fate (Vicious - WT3)

You now have a 40-60% increase in Critical Strike Damage, but your Non-Critical Strikes deal 15-20% less damage. Additionally, you generate 10% more Barrier and heal for 3-7 Life per second while your Barrier is active.

Revenge (Brutal - WT3) mitigates 10-20% of incoming damage. When using a Defensive, Subterfuge, or Macabre skill, the suppressed damage is magnified by 250% and detonates, causing up to 1360-2040 Fire damage to nearby enemies.

Prudent Heart (Brutal - WT3) grants immunity for 2.0-4.0 seconds after sustaining a single hit that surpasses 20% of your maximum Life. This immunity can only activate once every 110 seconds.

Determination (Devious)

Resource draining effects have a reduced effectiveness by 40-50%. Additionally, gain an increased Resource Generation of 3.0-8.0%.

Retaliation (Devious - WT3)

When a Crowd Control effect is removed from you, inflict 510-680 Fire damage on nearby enemies.

Calculated (Devious - WT3): Stuns enemies hit for 2 seconds after using 150-200 of your Primary Resource.

Malignant Pact (Wrathful): Cycle through a Malignant bonus every 20 kills. Vicious: Gain a 20% boost in Attack Speed.

Devious: Core and Basic Skills have a 15% chance to fully restore your Primary Resource.

Brutal: Every 21 seconds, gain a Barrier absorbing 85-102 damage.

Creeping Death (Wrathful - WT3): Your damage over time effects receive a boost of 30-40% for each unique Crowd Control effect affecting the target. Unstoppable monsters and Staggered bosses are hit with 110-130% increased damage from your damage over time effects.

The Barber (Wrathful - WT3): Critical Strikes and any ensuing damage within 2.0-4.0 seconds are absorbed by your target. Subsequently, the absorbed damage is unleashed upon nearby enemies. The stored damage accumulates at a rate of 10% per second.

Barbarian

Focused Rage (Vicious)After spending 100-60 Fury within 2 seconds, your next Non-Basic Skill’s Critical Strike Chance is increased by 20-30%.

Resilient Revival (Brutal) - When your life is between 40-60%, you gain an additional 50-60% healing from any source.

Reckoning Velocity (Devious) - Your abilities have a chance of 20-30% to topple all foes for 1.25 seconds if their attack speed exceeds 35-20%.

Ignoring Pain (Wrathful - WT4)Incoming damage has a 5-15% chance of being ignored and instead Healing you for 17-68.

Rogue

Cluster Munitions (Vicious)Lucky Hit: There is a maximum of 20% probability that 3 Stun Grenades will be released, causing 26-32 Physical damage and stunning enemies for 0.50 seconds.

Trickery (Brutal)Upon utilizing a Subterfuge Skill, a precarious Shadow Decoy Trap will be left behind to taunt enemies. After 6.0 seconds, the Shadow Decoy Trap will detonate and inflict 680-1020 Shadow damage. This occurrence is restricted to once every 5 seconds.

The Mighty Banquet (Vengeful)Each Follower consumes 1.0-2.0 Essence every second but inflicts 50-75% amplified damage. In the absence of any Followers, this boost is bestowed upon you, draining 5 Essence per second.

Druid

The Moonrage (Vicious) Kills grant a 5% chance of summoning a Wolf Companion to accompany you for a duration of 20-30 seconds. You also receive a +3 bonus to Wolves.

The Agitated Winds (Brutal) Automatically activates Cyclone Armor when facing 8-13 nearby enemies. This ability can only trigger once every 10-20 seconds.

Inescapable Force (Deceptive): Draws in up to 30-50 distant enemies towards you when you have an Ultimate Skill activated.

The Unrestrained Creature (Enraged): Upon being struck with a Stun, Freeze, or Knock Down effect, there is a 40-60% probability of triggering Grizzly Rage automatically for a duration of 3 seconds.

Sorcerer

Tal'Rasha (Vicious)For each unique element you deal damage with, you deal 7-12% increased damage for 3-10 seconds.

Spellbreaking (Brutal): Upon receiving Elemental damage, acquire a 20-40% Resistance against that specific element for a duration of 5 seconds.

Spite (Devious): When affected by a Crowd Control effect, there is a 20-40% probability that the corresponding enemy and nearby enemies will also experience the same effect for a period of 3 seconds.

Omnipower (Wrathful) Core Skills consume all of your Mana to launch projectiles. Each additional 45-35 Mana consumed triggers an extra projectile, increasing the damage by 3.0-5.0%.
