Killing Diablo 4's Butcher has the possibility of rewarding players with a mount cosmetic known as the Butcher's Cleaver. However, the inclusion of this cosmetic has caused controversy among the player base. The issue lies in the small size of the cleaver when attached to the horse, making it difficult to notice, especially when combined with other mount cosmetics. Although this matter has been extensively discussed, some players have only recently realized that they already have the item in their inventory after battling the Butcher.

Rarely seen, the Butcher can be an intimidating presence for players exploring the early stages of the game. This can lead to players either running away or being trapped, resulting in their character's demise, which is particularly worrisome in Diablo 4's permadeath Hardcore mode.

Although players can eventually triumph over the Butcher, it may take considerable time and the ensuing relief may overshadow the discovery of a cosmetic drop.
