However, as the story progressed, fans learned that he was more than what meets the eye. Yuji is actually a Cursed Womb Death Painting, one of nine and likely the most "perfect" of the lot.

Death Painting Wombs are Cursed Objects created from the union of Cursed Spirit and Human blood. Fully incarnated Death Painting Wombs are ranked similarly to a Special-Grade Cursed Spirit. Their flesh and blood bodies liken them to normal humans and even sorcerers have trouble distinguishing them from Cursed Spirits or Curse Users.

Kenjaku had taken over Yuji's mother's body, Kaori Itadori (chapter 143). During this time, he changed Yuji in order for him to become a vessel for Ryomen Sukuna. This explains Yuji's extraordinary physical abilities and his capability to see Cursed Spirits.

Judging by what has happened in Jujutsu Kaisen so far, Sukuna doesn't seem to know who Yuji really is. To Sukuna, Yuji was just his vessel until he took over Megumi Fushiguro. However, there appears to be some sort of connection between Yuji and Sukuna.

In Jujutsu Kaisen chapter 248, Sukuna finally recognizes Yuji. He was annoyed by the fact that Yuji remained firm in his beliefs. Despite being knocked down multiple times, he always got back up. He stood by his beliefs so strongly that he was willing to confront someone like Sukuna no matter what.

In the same chapter, Sukuna mentions that the Jujutsu High sorcerers reminded him of someone he knew "1000 years ago". The identity of this person remains a mystery. One possible theory is that this "someone" could be Sukuna the Sorcerer, before he transformed into Sukuna the Curse.

To put it simply, Ryomen Sukuna may not be familiar with Yuji Itadori's true identity. Even though he knows about Kenjaku's unethical use of Jujutsu to achieve his goals, it is doubtful that he knows who the boy really is.

Sukuna believed that Megumi would be a better fit for reincarnation. After successfully completing the process, Sukuna is now fully reincarnated and ready to cause trouble once again.

With Gojo and Higuruma possibly out of commission, Yuta injured, and Yuji trying to rejoin the fight, Maki Zenin could be the crucial factor for turning the tide in this chaotic battle.
