In the previous episode, viewers witnessed Izana's encounter with Shinichiro and learned about his growing animosity towards Mikey. Following Shinichiro's death, Izana sought revenge against Mikey, but found himself overpowered. When he resorted to using a gun, Kakucho intervened, only to be shot by Kisaki.

Tokyo Revengers season 3 episode 12 preview hints at Kakucho getting gravely injured

It is highly probable that in Tokyo Revengers season 3 episode 12, titled Paradise lost, Kakucho will suffer severe injuries. It is important to remember that the conflict between the Tokyo Manji Gang and Tenjiku was essentially a fight between high school students for dominance and recognition.
Bringing a gun to the battle was already extreme, but Kisaki Tetta crossed the line when he shot Kakucho with it.
Kakucho's wound was serious and without immediate medical attention, his condition could deteriorate rapidly. With the battle happening in a remote location, there was little hope of an ambulance or emergency services reaching them in time. This meant that Kakucho's life was at risk from excessive bleeding.
Kisaki Tetta may end up shooting Izana Kurokawa

Struggling to shoot Takemichi, Kisaki Tetta ultimately took aim at Kakucho. It seemed that Kisaki was growing weary of his constant failures. Despite Kisaki's relentless pursuit, Takemichi continued to intervene and disrupt his plans.
Therefore, Kakucho's intervention in preventing Izana from harming Mikey may have pushed Kisaki over the edge, leading to his decision to shoot Kakucho.
Based on the preview image, it is apparent that Kakucho is still alive in the next episode. This may lead Kisaki to attempt to shoot him once again. This turn of events could potentially create turmoil in Izana's heart, prompting him to intervene and try to save Kakucho. In doing so, there is a possibility that Kisaki may accidentally shoot Izana Kurokawa.
Hanagaki Takemichi may go after Kisaki Tetta

After Kisaki Tetta shoots Kakucho, there's no way Takemichi Hanagaki will just stand by and watch. Despite his severe injuries, Takemichi may pursue Kisaki and attempt to finally defeat him. Now that Draken and Mikey are aware of Takemichi's time-traveling origins, they may go to great lengths to assist him.
Both Toman leaders have strong motivations to personally bring down Kisaki. Despite Mikey's apparent forgiveness towards Izana Kurokawa for his involvement in Emma's death, there is no way he will let Kisaki off the hook. Draken, whose girlfriend was Emma, feels the same way.
