Unlike previous Diablo games, the open world in Diablo 4 has a fixed and predetermined path, but it still offers a diverse online experience by dynamically connecting players across the same World Tier. While the events within Sanctuary's regions are randomized, the classic Diablo zone design remains reserved for its dungeons.

The dungeons in Diablo 4 are the core of its gameplay, offering players the chance to obtain legendary traits for the Codex of Power by completing certain dungeons. The endgame revolves around challenging Nightmare Dungeons for better loot and facing the emerging Helltides in the open world. However, to unlock these features, players must first prove themselves by completing the campaign on either Normal or Veteran difficulty and finishing the Capstone Dungeon on the highest difficulty available.
