Among all the loot drops in the game, unique quality gear is the rarest, with six exceptionally rare items classified as the rarest of the rare. Initially believed to be a myth, lead class designer Adam Jackson confirmed the existence of these items on Twitter, igniting players' excitement to uncover them in-game and be the first to do so. These exclusive items can only be obtained by defeating enemies at level 85 or higher, typically found in Tier 31+ Nightmare Dungeons on World Tier 4, making them accessible only in the late stages of the game. As of late, players have successfully discovered all six of these items, marking the end of the hunt for Diablo 4's most elusive treasures.
Harlequin Crest (Shako)
The Harlequin Crest, also known as the Shako from Diablo 2, holds the reputation of being the most infamous super rare item. It was the first of its kind to be discovered in Diablo 4, just a few days after the game's release. The lucky finder was a member of drlee_kor's community, a Korean Diablo streamer. The confirmation of this discovery came from Adam Fletcher, the global community director of Diablo 4, who shared the news on Twitter.
Apart from its rarity, what makes the Harlequin Crest so highly sought after are its extraordinary affixes that greatly enhance a player's build in the later stages of the game. Equipping this item grants a whopping 1025 Armor, a boost of +732-1359 Max Life, +4.4-10.0 percent Cooldown Reduction, +5.0-12.0 percent Resource Generation, and +20-28 All Stats. Additionally, its unique affix provides an additional +10.0-20.0 percent Damage Reduction and a remarkable +4 to all Skills, making it an incredibly versatile helmet for any type of build.
Andariel's Visage
Following the unveiling of the Shako shortly after the release of Diablo 4, the remaining ultra-rare uniques remained undiscovered until recently. Among these, Andariel's Visage was the next to be found, shortly after confirmation by Adam Jackson of their existence in-game. It was Lithie, a player of Diablo 4, who made this discovery and showcased it during a livestream by Polish streamer NadinWins, leaving viewers astonished.
Similar to the Shako, Andariel's Visage is an incredibly versatile helmet designed to significantly enhance the wearer's survivability. In addition to providing 1025 Armor, this helmet boasts affixes such as a 4.4-10.0 percent Attack Speed increase, +24.5-45.5 percent Poison resistance, +10-18 All Stats, and +1.3-2.0 percent Life Steal. The unique affix of Andariel's Visage grants the wearer a Lucky Hit chance to trigger Poison Nova, inflicting 10,077 Poison damage over 5 seconds. This ability proves highly effective in draining the life of enemies and replenishing the wearer's health through its Life Steal affix.
The Grandfather
After the recent discovery of Andariel's Visage, another incredibly rare unique item known as The Grandfather has been found by a skilled Necromancer from China. The item's existence has been confirmed through a screenshot shared on Reddit. Considered to be one of the finest weapons in the game, The Grandfather is a highly sought-after late-game treasure. Surprisingly, this powerful weapon was obtained from a random event chest during a Helltide, despite complaints from players about Diablo 4's current stat rolling system.
The Grandfather is a mighty two-handed sword that can be wielded by any class in Diablo 4. It proves to be a formidable weapon when aiming to maximize damage output. What makes it even more impressive are its exceptional affixes. These include the ability to deal 1987-2981 Damage per Hit at a rate of 1.0 hits per second, a +35 percent increase in Critical Damage, a +28-56 percent boost to All Damage, an addition of 1463-2717 Maximum Life, a bonus of +40-72 to All Stats, and the valuable attribute of ignoring durability loss. In addition, The Grandfather possesses a unique affix that enhances Critical Damage by +60-100 percent. Furthermore, it possesses a special property that allows the other affixes on the weapon to roll at a higher value than usual.
Ring of Starless Skies
The third and final batch of exceptionally rare and distinctive items were discovered in a remarkably short period of time. The initial find was the Ring of the Starless Skies, which was promptly claimed by a Necromancer operating below Level 60 in a Tier 31 Nightmare Dungeon. This ring boasts several enhancing attributes, including a 25 percent increase in Cold Resistance, a 25 percent increase in Fire Resistance, a +3.2-6.0 percent chance of landing a lucky hit, a +1.8-5.0 percent chance of scoring a critical hit, a 14-19 percent increase in critical damage, and a 12.5-19.5 percent enhancement to Core Skill Damage. Moreover, it contains a unique aspect that diminishes the cost of the subsequent Core Skill cast by 8-12 percent. Through consecutive use of Core Skills, this reduction can reach a maximum of 40 percent.
Doombringer
Player ViZiOnZ333 uncovered the rare Doombringer sword while playing as the Diablo 4 Rogue. This weapon possesses exceptional strength, imbued with affixes that bestow upon its wielder the ability to inflict 903-1355 Damage per Hit (at a rate of 1.1 hits per second), a substantial 17.5 percent increase in Critical Damage, heightened Core Skill Damage ranging from 12.5-19.5 percent, augmented All Damage between 10.5-17.5 percent, a formidable 12-16 percent augmentation to Maximum Life, and the added bonus of Lucky Hit: a 5 percent chance to restore 523-941 Life. Furthermore, this sword harbors a unique affix that amplifies the odds of landing a Lucky Hit, granting an additional 15-20 percent chance to deal a staggering 5,822 Shadow damage while simultaneously applying a 20 percent damage reduction to nearby adversaries.
Melted Heart of Selig
The final unique item to be found in the sixth chase is the Melted Heart of Selig. This powerful amulet was discovered by a Necromancer in the Blind Burrows, completing the hunt for these rare Diablo 4 items. Wearing the Melted Heart of Selig grants the wearer various bonuses, including 18.5 percent resistance to all element types, +12.5-19.5 percent Core Skill damage, +16.5-23.5 percent damage while in a healthy state, +5.0-12.0 percent resource generation, and +10-18 All Stats. Additionally, the unique affix on the amulet offers a high risk/high reward effect. It grants a +30 percent maximum primary resource, but for every 1 percent of life lost, it drains 8-3 of that resource, maintaining a healthy state.
