Named Bloodthirst, the legendary dagger is in the possession of Orin, a malevolent shapeshifter and loyal follower of Bhaal, the Lord of Murder. The blade carries a strong mystique, much like its wielder, as Orin seamlessly incorporates it into her every scene. Originally, the blood-red, asymmetrical blade belonged to Bhaal himself when he was still mortal.

To acquire the dagger, players must progress to Act 3 of Baldur’s Gate 3’s storyline, with the sole requirement of ultimately engaging in battle against Orin within the Bhaal Temple. However, players have the freedom to accomplish everything leading up to and following that point in their own unique way, including the option to embrace the Dark Urge. Once the formidable shapeshifter Orin is defeated, the coveted weapon can be claimed without hinderance.

Orin does not rely on Bloodthirst during battles, but instead utilizes Crimson Mischief, a short-sword, and Blade of the First Blood, a dagger that bears a striking resemblance to Bloodthirst. If players defeat Orin and loot her body before concluding the fight, typically by keeping her cultists alive until then, the dagger will remain unchanged and it will be impossible to obtain Bloodthirst.

However, for narrative purposes, the dagger transforms into Bloodthirst when looted from Orin's body after the fight concludes. The only distinctions between these two versions are that when used as an off-hand weapon, Bloodthirst provides a +1 AC rating, while Blade of the First Blood offers a -1 Armor Class rating. Additionally, First Blood inflicts both Necrotic damage and Piercing damage. There are no disparities in their appearance. Both Bloodthirst and Blade of the First Blood are exceptional choices, so it ultimately depends on the player's personal preference.

Bloodthirst, a Light weapon, can be dual-wielded by select classes such as rogues. It pairs well with Crimson Mischief, but its true advantage lies in the bonus to the critical stat and the addition of a Divination cantrip. During turn-based combat, landing a critical hit can be the difference between life and death.
