Tae-sang risks his life in Gyeongseong Creature Part 1
Chae-ok and most of the characters manage to survive and flee from Onseong Hospital, while Tae-sang remains behind to distract attention. Gyeongseong Creature Episode 7 marks the start of the conclusion, opening with future scenes before shifting to the present. After aiding the prisoners in escaping, Chae-ok and Tae-sang initiate the plan to leave the hospital. As the prisoners make their way to a camouflaged armored truck through the windows, Ichiro pursues them in an attempt to halt their escape.
When a soldier captures them, Yeong-gwan has no option but to take his life, alerting Ichiro to their whereabouts in the process. With Yeong-gwan's assistance, Chae-ok and Tae-sang brace themselves for the incoming gunfire. In the chaos of the exchange, Yeong-gwan sustains fatal injuries. He implores Chae-ok to remember him and provides her with his Korean name, address, and tag to give to his mother. Tae-sang and Chae-ok ensure that the coast is clear before Chae-ok makes her way down to the truck.
However, Tae-sang instructs his friend to leave first and emphasizes the importance of sticking to the predetermined departure time, no matter what. He has made the decision to stay behind and create a diversion. In accordance with Wol-daek's instructions, Tae-sang's butler is also told to depart without him if necessary. As they execute the plan, Tae-sang confronts the soldiers head-on and is injured in the process. He manages to escape through the vents and ends up in a room filled with skeletons, unaware of its location.
The najin parasite leaves Onseong Hospital
At his return to the town, Wol-deak uses the porcelain vase to negotiate a deal for Joon-take’s release while Myeong-ja is safely returned to Ishikawa. A festive gathering in the town center serves as a distraction to camouflage the survivors as local townspeople. Meanwhile, at Onseong Hospital, Lieutenant General Gato discovers that someone has consumed the water contaminated with the najin parasite. Chae-ok and her father successfully make their escape.
Before the credits roll, it is disclosed that Myeong-ja, who is pregnant, is the one who ingested the water and is now carrying the najin parasite.
Lieutenant General Gato concludes that either Chae-ok, Tae-sang, or Myeong-ja consumed the water. The scene then shifts to Ishikawa's wife being informed that her husband will not be returning home, indicating that her attempt to eliminate his mistress has failed. Ichiro visits her, revealing that she is the true mastermind behind Onseong Hospital.
He presents her with the successful test subject reports, including drawings of Chae-ok's mother as the monster. The final scenes of Gyeongseong Creature Part 1 leave Gato pondering who drank the water, Myeong-ja sleeping with Ishikawa, Chae-ok standing in a desolate town, and Tae-ok being alive.
Struggling to walk through the corridors, he wakes up and places his bloody hand on the wall, overlapping the handprint of the monster. The scene then shifts to an older woman at home, the same woman who went to Tae-sang’s pawn shop to exchange her sewing machine for information about her son. She discovers a package on her doorstep containing her sewing machine with a name tag – Yeong-gwan's.
Before the credits, a scene unfolds to reveal the truth. As Myeong-ja sleeps, a close-up exposes her consumption of najin, and its journey to her brain. The conclusion of Gyeongseong Creature Part 1 leaves open the question of how the najin will impact her unborn child.
