In 2013, Benita Alexander was assigned to create a documentary called A Leap of Faith, showcasing his achievements in the medical field. The two formed a romantic relationship, leading to their engagement.

However, Macchiarini's initial groundbreaking work in organ transplants soon came under scrutiny due to allegations of unethical medical practices and research.

The investigations uncovered that Macchiarini had conducted experimental surgeries without the appropriate authorization or sufficient patient consent. A number of his patients who received trachea transplants experienced serious complications or fatalities, resulting in accusations of medical wrongdoing.

Furthermore, certain grandiose statements he made to Alexander, such as being the personal surgeon of then-POTUS Barack Obama, were called into doubt.
