In a game centered around a monumental conflict between Heaven and Hell, with demons and the undead as the prominent characters amidst a gruesome atmosphere, there are bound to be unsettling quests. This is where the Diablo franchise established its reputation, and Diablo 4 carries on this legacy.

The quests within Sanctuary can be truly terrifying for a variety of imaginative reasons, although certain ones surpass others in terms of sheer dread. It's not solely about graphic violence, but rather the unsettling fear of the unknown and the misconceptions that arise from it.

7 Picking Through The Bones

The Whispering Tree always evokes a spine-chilling sensation for players. "Picking Through The Bones" is a crucial part of the campaign quest chain, specifically in Act V: Secrets Bartered, Fates Sold, where the terror escalates. It follows the events of "A Cold And Lifeless Shore" and is succeeded by "Beneath The Wine Dark Sea."

This quest takes on the essence of a thrilling mystery, as players must navigate through eerie clues and unravel the enigma of a cursed amulet. The initial stage involves straightforward investigative tasks, followed by a harrowing mission to scour funeral pyres and corpses for fragments of a journal.

The path ultimately leads to the ominous Light's Refuge dungeon. While it is not mandatory to conquer it in order to locate the amulet, it becomes essential to unlock the door that leads to the latter half of the dungeon, where Aldara patiently awaits.

4 Malady Of The Soul

This is the infamous exorcism quest series, resembling a thrilling occult tale. It consists of three quests, where the player assists Mother Octavia in her endeavor to cure individuals possessed by demons instead of resorting to execution. The traditional method of burning them at the stake has not been well-received, prompting Octavia to seek an alternative solution. She can be found waiting on the southeastern side of Kyovashad.

The initial exorcism proceeds smoothly, albeit accompanied by the expected macabre elements, but the second attempt ends tragically. The person Octavia is attempting to save freezes and shatters into countless icy fragments. In the final quest, Octavia becomes the afflicted victim in need of rescue, with the player assuming the role of the exorcist.

3 The Woodsman Of Nevesk

Introducing a thrilling quest chain that has gained a reputation for keeping players on edge, this chilling narrative revolves around an axe possessed by an intense force. Initially appearing to be a simple tale of a serial killer, players are soon taken aback as they realize that the axe itself is haunted, with the spirit of a deceased woodsman latching onto anyone who wields it.

Embark on the first step of this enthralling journey by interacting with an NPC named Magdalena and initiating the quest titled "The Woodman's Axe." As the player locates and retrieves the axe, the story unfolds, progressing through subsequent quests such as "Chain of Possession" and "Legacy Unmade." Eventually, the player emerges triumphant, having vanquished the menacing Woodsman and possesses a magnificent new axe for their own use.

2 Unyielding Flesh

Lilith left a trail of pain and suffering as she traveled through Sanctuary, tempting humans to give in to their darkest and most forbidden desires. One of the side-quests, called "Unyielding Flesh," delves into one of these disturbing stories. The quest begins with the story of a betrayed wife and an unfaithful husband.

Even without the references to the Hellraiser movies, this quest is still haunting and unsettling. To embark on this quest, head to the small town of Yelesna, located southeast of Kyovashad in Gale Valley. Krystyna can be found on the southeastern side of town, seeking help to locate her missing husband whom she suspects has run away with another woman. The fate that awaits him, however, is far worse than death. Completing the quest rewards the character with a new dagger.

As if that weren't horrifying enough, players must then navigate their way to the foreboding Hallowed Ossuary, a haunting dungeon where the remains of Lord Avitus rest eternally, until they stir once more as an vengeful revenant. The completion of the quest occurs naturally upon triumphing over Avitus, for there are no surviving individuals to bestow the reward.
