Chapter 401 of My Hero Academia was expected to feature a collaborative fight between Stain and All Might against All For One (AFO). The latest spoilers, which were unveiled today, lived up to that expectation. However, the joint battle was not the highlight of the chapter.
In the preceding chapter, All Might employed various attacks to weaken AFO. Unfortunately, the villain's regression ability transformed him into a radiant child. Additionally, Hagakure experienced a revelation when she became fully visible and recognized that her intensified emotions frequently led to malfunctions with her Quirk.

According to My Hero Academia chapter 401 spoilers, the chapter is named “The Lunatic.”
The chapter starts with a flashback of Stain utilizing his Glider to travel towards the battle between AFO and All Might. By overhearing All Might's conversation with Iida and Shoto, whom he refers to as the "Scar kid," Stain comprehends the situation. As he approaches, Stain notices an increase in blood and debris. Upon closer inspection, he discerns that the blood does not belong to All Might.
Returning to the present in My Hero Academia chapter 401, Stain ponders whether All Might comprehends the significance of his own existence. Stain delivered his speech in Kamino with the sole intention of making All Might recognize the value of his life. All Might responds that he indeed understands, and the two launch a coordinated attack.
In the midst of the battle, Stain attempts to replicate All Might’s iconic phrase, "I Am Here" (Watashi ga kita) with a slight personal twist, "Ore ga kita." However, to his disappointment, this altered version fails to have any impact. According to AFO, Stain was the only one who managed to escape Tartarus on his own, indicating that the demon lord was aware of his plans against the villains and had taken various precautions. Realizing the need for more blood, Stain discovers that AFO has altered the composition of his blood through Antigen Modification.
It seems that AFO is experienced in such matters, as this is how he has been able to operate in secrecy for a considerable period of time. Suddenly, a mouth emerges from AFO's back and bites Stain, causing him to bleed heavily. AFO taunts him, suggesting that the replacement of his blood has cooled him down. The exact target of AFO's mockery is unclear in the spoilers for My Hero Academia chapter 401, but it is likely directed at himself.
AFO taunts him, mocking the significance of Stain to Tomura and the league, deeming him useless now. Stain, however, sees a greater purpose for all of them. All Might had taught him the true meaning of being a hero, and now he desperately prays to his idol to emerge victorious as AFO collides with him, causing blood to gush out. Presumably, AFO steals his ability.
AFO swiftly redirects his aggression towards All Might, who was approaching him in flight. Fortunately, the remnants of Hercules intervene to shield him and provide encouragement to keep fighting. As the car is completely wrecked, All Might experiences visions of Nana Shimura and Sir Nighteye. It dawns on him that his body is resiliently trying to preserve his existence, even though he can no longer utilize his legs.
All Might remains alive and defiantly challenges AFO to confront him. However, a subsequent police report reveals that AFO has managed to approach Shigaraki and infiltrate U.A. by exploiting his mysterious Warp teleportation Quirk. The final panel of My Hero Academia chapter 401 unveils a chilling image of AFO gazing upon the descending U.A. island.
